的 Care Management for High-Risk Pregnancies (CMHRP) Program, formally known as Obstetrics Care Management (OBCM), is comprised of a group of committed and passionate social workers and nurses. 的 team of social workers and nurses are also known as Pregnancy Care Managers. Pregnancy Care Managers work with you and your prenatal care provider to ensure that you receive the best care while you are pregnant and after you deliver your baby. 孕期护理管理 services are available to pregnant women enrolled in North Carolina 医疗补助计划 statewide, and to a limited number of low-income, uninsured pregnant women in some counties.

Benefits of being enrolled in the CMHRP Program:

Pregnancy Care managers work collaboratively with prenatal care providers to assure that both mom and baby experience a healthy pregnancy and birth outcome. Our goal is to help you have a healthy pregnancy and healthy baby.

Your Pregnancy Care Manager may assist you by:

· Working closely with you and your doctor to meet all your needs
· Helping you get connected to services that you may need such as: transportation, 食物, 医疗保健, 或住房
· Meeting with you after you have been in the hospital or emergency room to see how you are doing

· Assisting with referrals to other statewide programs such as WIC, fn, 医疗补助计划, 和计划生育
· Answering any questions that you may have

Your care manager will talk with you on the phone, meet you at your prenatal visits, or meet you in your home or at a location that is convenient for you.

在你第一次看产科医生的时候, your provider will complete a pregnancy risk screening in order to identify if you have any social barriers or health concerns that may prevent you from having a healthy pregnancy.

· Have a history of pre-term labor
· Have a history of low birth weight
· Are pregnant with more than one baby
· Have an unsafe living environment
· Have started prenatal care late
· Have missed two or more prenatal appointments
· Have been to the hospital during the pregnancy
· Have fetal complications   

In addition to the Pregnancy Risk Screening, participants may be referred to the CMHRP program by their Prepaid Health Plan (PHP). Contracted PHPs include: WellCare, 健康的蓝色, 联合健康保健, AmeriHealth Caritas and Carolina Complete Health.

CMHRP accepts self-referrals and community referrals as well.
