的 Association of State and Territorial Health Officials defines population health as the health outcomes of a group of individuals including the distribution of such outcomes within the group with three main components- health outcomes, health determinants and policies.
的 Durham County Department of Public Health Population Health Division focuses on community engagement, data, quality improvement, workforce development, strategic planning, accreditation and policy development.
的 division will use many strategies to improve health outcomes of Durham County residents, centering data and equity such as:
- Community Centered Approaches
- Community-driven health priorities (Community Health Assessment & listening sessions/Partnership for a Healthy Durham)
- Integrate residents’ stories to contextualize population health statistics
- Expanding User-Friendly Public Data
- Develop data dashboards that highlight existing health inequities and progress in addressing community-driven health priorities
- Develop infographics and reports to disseminate data on health inequities and priorities
- Policy Development
- Intentional focus to increase policy outcomes that maximize population health
- Engage community partners in conducting policy analyses to determine which polices will maximize population health
- Community Engagement:
- Engage the community in an authentic way that acknowledges their expertise
- Create a community advisory council
- Obtain National Public Health Department Accreditation
- Meet the requirements of the many population health and equity standards
